  I hereby give approval for my child's participation in all activities at The Kenesky Goaltending School and agree to save harmless and keep indemnified The Kenesky Goaltending School and it's employees. I also assume all risks and hazards incidental to such participation including medical and dental expenses. Refunds will not be honoured after June 15, 2008. 10% administration charge on all refunds before June 15, 2008.
